focus group questionaire

We are now planning to get our focus groups together and have so far got two set up
year 11 set 1 all girls
year 11 set 3 mixed
we have created a questionnaire for the students to do so that we get feedback from them and also we will record us doing so for proof and take photos if possible when needed showing they are working. This is our questionnaire and we will give this to every student to fill out and hopefully get some great feedback!!!!
